Privacy policy for maternity care professionals

In our Privacy Policy, we use several definitions (these definitions can be used in singular and plural):

Policy: this privacy policy document.

Privacy legislation: General Data Protection Regulation (GDPR) (as of May 25, 2018: the General Data Protection Regulation).

1. Explanation of the Regulations

Maternity care professionals are not allowed to process the data you provide them without proper authorization. Privacy legislation is designed to protect individuals’ privacy. This legislation limits the permitted use of your personal data by others. Based on this law, maternity care professionals have an obligation to:

• inform you about how your data is processed by them and for what purpose,

• disclose who can access the data,

• seek your consent for processing certain data.

Maternity care professionals value your privacy. Therefore, this policy provides an explanation of how we handle your data, the purpose of its use, and the specific data processing activities for which we explicitly need your consent.

2. The personal data that maternity care professionals use and the purpose of its use.

Maternity care professionals may process (potentially) your personal data if you become or are a client of maternity care professionals or if you contact us via the application form. Maternity care professionals collect your name, address, phone number, date of birth, place of birth, email address, information regarding your insurance company, policy number, and your citizen service number (BSN).

These data enable us to:

•Financially and administratively handle the agreement clients make with maternity care professionals;

•Provide our services;

•Further develop/optimize our services.

3. Provision of personal data to third parties

Maternity care professionals do not disclose personal data to individuals or companies outside the maternity care professionals organization, unless:

•It is required by a legal regulation;

•It is necessary for the execution of an agreement that maternity care professionals have made with you;

•You have given your consent for it.

4. Obligations/secure access/confidentiality/retention period

• Maternity care professionals process your personal data in accordance with the law. This means that the data are only processed for the purpose they were obtained and in a proper and careful manner, in accordance with the law and this policy.

• Your personal data can only be accessed by the employees of maternity care professionals, unless otherwise specified in this policy. All your personal data are secured by maternity care professionals to prevent unauthorized access. The security measures include:

• Each employee has a personal password to log into the digital system.

• Employees of maternity care professionals are bound by confidentiality regarding all personal data provided to maternity care professionals.

• Your personal data will not be stored longer than necessary.

5. Your rights as a data subject

• Right to information: the right to know if and for what purpose your personal data is being processed;

• Right of access: the right to access and obtain a copy of that data, as long as it does not infringe upon another person’s privacy;

• Right to correction, supplementation, or deletion of data if necessary (right to correction and deletion). The right to request (partial) deletion of your data can only be fulfilled if retaining the data is not significantly important to others and if there is no legal obligation to keep the data;

• Right to object: the right to object to the processing of your data in certain cases.

If you wish to exercise your rights, you can indicate this via email to info@kraamzorg-professionals.nl. If your request is denied, the reasons will be explained to you. One reason may be that your file contains information that is important or relevant to others. You will receive a response from maternity care professionals within one month of receiving your request.

If you have a complaint about the processing of your personal data, you can also contact maternity care professionals, and they will work with you to find a solution.
